Improvements

• Improved speed of command-line driver due to a broadcast window message.
• The named-pipes driver now handles step-in / step-over when the process is not running (starts the target and runs to the canonical entry point).
• Clients will now receive notifications to source location changed when transitioning to the disassembly view (i.e., no source available). In this case the filename will be empty and line number set to zero.
• Moved "Display Name" below command, arguments, and working directory.

Bug Fixes

• Fixed crash when toggling "Reload Text" option when no text windows are opened.
• Fixed "run-to-cursor" to work with source locations in unloaded DLLs.
